Job Description:
The Hackensack UMC Part Time (24 hrs/wk) Hospital at Home Nurse
Practitioner will serve in a dual capacity. The APN will take a
lead role in identification and assessment of potential Hospital at
Home referrals, in collaboration with the Virtual Hospitalist. The
APN must be committed to providing high-quality, evidence based,
patient centered care to a diverse patient population in a
non-traditional setting. Additionally, the APN will conduct in-home
patient visits to Hospital at Home patients. (2) 12 hr shift
requirement each week.
